Consent for Treatment Form * - Medical Release Consent for Treatment - this form is automatically part of all CCYL regular season registrations, but is being posted here so that all Travel Teams may also access it easily.
Manager Application * - CCYL is only as good as the managers, and we never have enough of them. You don't have to have extensive baseball or softball knowledge or experience, though that certainly helps. But you do have to have a positive attitude and want to help shape the lives of our community's youth through enjoying a wonderful game. Apply within.
Manager Evaluation Questionaire * - The Manager's Evaluation Questionaire should be submitted by all parents/guardians at the end of every season. The CCYL BOD will use this vital information to help determine future managerial positions.
